If you are an employer and are interested in hiring co-ops for the upcoming terms, please fill out the needs form and fax it to our office or send as an attachment through email.

Student Performance Evaluation Form

At the close of the work term the employer is responsible for:
Completing a written evaluation
Returning the evaluation to our office (by fax, U.S. mail, or e-mail)

We ask that you complete the Student Performance Evaluation Form found below.
We strongly recommend that you discuss the student’s evaluation with him/her in person before the student leaves your work site.
The evaluation also mandates a signature by both the supervisor and the student.

A copy of the evaluation is placed in the student’s co-op file.
