If you are an employer and are interested in hiring co-ops for the

office or send as an attachment through email.

Performance Evaluation Form
At the close
of the work term the employer is responsible for:
Completing a written evaluation
Returning the evaluation to our office (by fax, U.S. mail, or e-mail)
We ask that
you complete the Student Performance Evaluation Form found below.
We strongly recommend that you discuss the student’s evaluation with
him/her in person before the student leaves your work site.
The evaluation also mandates a signature by both the supervisor and the student.
A copy of the
evaluation is placed in the student’s co-op file.
